360 Process: Actions Needed

In order to facilitate the 360 process, LifeWorks Coaching & Training needs you to complete these 3 steps.

Step 1: Go to https://www.surveymonkey.com/r/lifeworks360 where you will share the email addresses of the people you are inviting to participate in your 360. Please include your leader, at least 5 peers or colleagues (this can include external stakeholders or customers), and all direct reports. You do not have to identify their role. They will be asked to check a box to identify their role.

Step 2: Once you have completed Step 1, please copy the email template below and send to your responders letting them know to expect the survey link. Feel free to add a personal note, either at the beginning or end of this message. Please copy [email protected] so she knows it is time to send the survey.

Step 3: Please complete the 360 by rating yourself – watch for the link as noted below from “[email protected] via surveymonkey”.

SUBJECT: Your input needed: 360 Survey coming

I am participating in a leadership development opportunity and I am working with a coach to grow as a leader. As part of that process, I will be completing an assessment that will help me understand my leadership styles, strengths, weaknesses, and impact.

An important part of this assessment is a survey process that measures how others perceive me.

I would greatly value your participation in this process. In the next few days, please watch for an email from [email protected] via surveymonkey.com which will contain a link to an anonymous 10-minute survey. 

At the end of the survey, you will be asked to identify your role – next level leader, peer/colleague, or direct report. Your answers will be combined into a composite report of 4 or more people so please know that I will not be able to identify what you have said. If there are not enough responses in either the peer/colleague or direct report categories, they will be combined to ensure this is a confidential process. Only the ratings from my next level leader will be known to me.

I am looking forward to getting the results of this survey so please be honest, open and comprehensive in your responses.
